You should sign up for our Member mobile app, myDentaQuest Texas, or our secure Member website! Once you register you can get copies of your ID card, check benefits and change your Main Dentist 24/7.

Help when you need it: Our Member service team speaks Spanish and English. We are ready to help 8 a.m. to 6 p.m. Central time. You can also use our secure Member website 24/7. The website can give you information on benefits, a copy of your ID card or let you pick a new Main Dentist.
Reminders for care: We will also reach out and let you know when it’s time for your child to visit the dentist. We can even help you set up the visit.
Dentists close to home: You can pick your child's dentist from our network. And if they don’t already have a dentist, we can help find one. If you ever need to pick a new Main Dentist, we can help with that too. You can even go online and change it anytime.

If your child is able to get Children's Medicaid, there is no cost to you for covered services.
If your child is able to get CHIP, your enrollment fee and co-pays will be based on your family’s income. Enrollment fees are $50 or less per family, per year. Co-pays for covered services like doctor visits and prescriptions range from $3 to $5 for lower-income families and $20 to $35 for higher-income families.

For Medicaid Members ages 20 years old and younger and CHIP Members ages 18 years old and younger. Download and register for the DentaQuest Mobile App within 30 days of enrollment. Limit of one gift card per household registration per lifetime.
For Medicaid Members ages 20 years old and younger and CHIP Members ages 18 years old and younger. Members who complete the risk assessment, qualify and enroll in the DentaQuest Case Management Program. Limit of one gift card per family per lifetime.

Medicaid or CHIP Members who receive four fluoride treatments (2 per year), four sealants (2 per year) and four oral evaluations (2 per year) within a 24-month period. Members can earn up to $100 in rewards for receiving important preventative services. For Members ages 3 to 10 years old. Limit one gift card per Member per lifetime.
*Gift Card can be used to purchase:
• Oral health supplies such as toothbrushes, toothpaste, floss and mouthwash.
• Baby care items such as diapers, bottles and formula.
• School supplies and clothes.
To join Medicaid or CHIP, the first step is to qualify for benefits. To do that, you’ll need to visit YourTexasBenefits.com. At the site, you can also find out about medical benefits, WIC and SNAP food benefits, and cash assistance.
To qualify a child must be:
• Age 18 or younger (children up to age 20 can qualify for Medicaid in some cases).
• A Texas resident.
• A U.S. citizen or legal permanent resident.
What if I am not a U.S. citizen?The state does not ask about the citizenship or immigration status of the parent. Only the child must be a U.S. citizen or legal permanent resident in Texas to be considered for Children’s Medicaid/CHIP coverage.
Once you qualify for benefits, you can call the enrollment broker to pick your plan. Be sure to tell them that you choose DentaQuest for dental services.
